REQUEST A REPEAT PRESCRIPTION ?
If you need regular medication, we can issue repeat prescriptions
without seeing you each time.
When you first register at the practice, you will need to see the
doctor to set up your repeat medications. A computer-generated re-order form will then accompany each
prescription.
To request a repeat prescription, use your re-order form in
any of the following ways
• Drop it into the repeat prescription box at your surgery
• Complete our online repeat prescription

• Post it to your surgery (addresses on the Practice Area Map page)
• Fax it to your surgery (fax numbers on the Practice Area Map page)
For safety reasons, we cannot take
telephone requests for repeat prescriptions. We can post your prescription to
you if you supply a stamped, addressed envelope, but you may also collect prescriptions
in person from the surgery during opening hours Monday to Friday.
Please allow a minimum of two working days (48 hours) between
the request and collection.
From time to time we will ask you to see the doctor or specialist
nurse to review your treatment and update your repeat prescription.
