How to registerIf you wish to register as a patient with the Practice, you will be required to provide the following documents. If you are unable to provide these documents please speak to a member of our Patient Services Team We can only take registrations from Monday to Friday, 9am – 12 noon and 2pm – 5pm. 1) Proof of Address

Please note, if you have been previously registered with the NHS under a different name, then we will need to see proof of your change of name eg Marriage Certificate or Deed Poll. 3) Completed Registration forms
To assist us in obtaining your previous medical records, please ensure that you complete all sections of these forms. If you have changed your name or any other details then please let us know. If you have been out of the country or in the Armed Forces for any period, please note the dates on the form 4) To complete your registration, a New Patient Appointment with a Healthcare Assistant must be attended. Please book this at registration. Temporary Patients and Overseas Visitors A “Temporary Resident” is a patient who is already registered elsewhere in the UK, but staying for a temporary period within our boundary, eg visiting relatives or a student home from university. If you need to be seen as a Temporary Resident then you need to provide us with proof of identification (as above) and complete a Temporary Registration Form. Please note, we are unable to provide some services to Temporary patients eg Smear Tests, routine vaccinations, routine referrals to the hospital etc as these should all be dealt with by your registered GP. We are here to assist with urgent medical care. Overseas Visitors from countries that have a reciprocal agreement with the NHS, may be treated within the NHS for Urgent and Immediately Necessary Treatment. Again, we will need proof of your identification and eligibility for treatment. Anyone visiting the UK from a country without an NHS reciprocal agreement, requiring treatment that is not “Immediately Necessary” can register as a Private Patient and pay the relevant fees – full details are available from our Patient Services team. Practice boundariesWe will register anyone living within our practice boundary. For a map of the practice boundaries click here. As no-one is automatically registered with a surgery, it is important that both you and all members of your household register at the same time. We recognise the important role of carers in looking after dependent relatives; please make it known if you are caring for someone. Continuity of careWhen you register we will confirm with you which of our three surgeries you will attend. We will let you know who will be your usual doctor, and, for continuity of care, we would encourage you to consult the same doctor as far as possible. Your first appointmentWhen you register with the practice you will be asked to complete a health questionnaire (one for each member of your household), and you will also need to come to a ‘new patient appointment’ with a nurse or healthcare assistant. This 10-minute appointment completes your registration and introduces you to the practice. This is an excellent opportunity to have a routine health check and will also provide the surgery with some details of your medical background. Changing your detailsIf you move, or if you change your telephone number, or if you change your name, please inform us as soon as possible. If you move outside the practice boundary, the Thames Valley Primary Care Agency (Telephone 0118 918 3333) will be pleased to give you details of surgeries in your new area. |
